Title V / SB 435

The completed Title V & SB 435 forms are due in the Athletics Office on or before Monday, November 9.  All Assistant Principals responsible for Athletics must complete and return this form to the Athletics Office before the due date.  Title V requires that coaches complete a Fist Aid and CPR course, and SB 435 legisation identifies coaches who need the one semester Physical Education Credential waiver to have the roll book for an athletic class.
